Medication Management
Information for Master Trainers
To become certified as a Master Trainer you must mail to VCU Department of Gerontology the following:
- A complete Master Trainer Application Form
- Proof of licensure (copy of license) as a physician, pharmacist, or nurse
- Resume citing evidence of teaching/ training experience
- Check or Money Order for $50.00 made out to the Department of Gerontology. (Do Not Send Cash)
- Certificate will be mailed within three (3) weeks
*Your certification will expire three (3) years from the date of approval
To renew Master Trainer Certification you must mail to VCU Department of Gerontology the following:
- A complete Master Trainer Renewal Request Form
- Proof of licensure (copy of license) as a physician, pharmacist, or nurse
- Check or Money Order for $50.00 made out to the Department of Gerontology. (Do Not Send Cash)
- Certificate will be mailed within three (3) week
Information for Facility Trainers
To become certified as a Facility Trainer you must mail to VCU Department of Gerontology the following:
- A complete Facility Trainer Application Form
- Proof of licensure (copy of license) as a physician, pharmacist, or nurse
- Evidence of successful completion of the facility train the trainer course conducted by an approved Master Trainer (copy of the signed certificate)
- Check or Money Order for $25.00 made out to the Department of Gerontology. (Do Not Send Cash)
- Certificate will be mailed within three (3) weeks
*Your certification will expire three (3) years from the date of approval
To renew Facility Trainer Certification you must mail to VCU Department of Gerontology the following:
- A complete Facility Trainer Renewal Request Form
- Proof of licensure (copy of license) as a physician, pharmacist, or nurse
- Check or Money Order for $25.00 made out to the Department of Gerontology.(Do Not Send Cash)
- Certificate will be mailed within three (3) weeks
Upon completion of facility training you must mail to VCU Department of Gerontology the following:
- A complete Medication Management for Medication Aides Class Roster with all participants who have successfully completed the medication management training
- A complete Medication Management Individual Training Record signed by the instructor
- A complete Trainee Task Record signed by the student and instructor
- Check or Money Order for $7.00 made out to the Department of Gerontology for each participant.
(Do Not Send Cash) - Certificate will be mailed within 5-10 business days
Information for Medication Aides
To become certified as a Medication Aide you must mail to VCU Department of Gerontology the following:
- A complete Medication Management Individual Training Record signed by the instructor
- A complete Trainee Task Record signed by the student and instructor
- Check or Money Order for $7.00 made out to the Department of Gerontology for each participant.
(Do Not Send Cash) - Certificate will be mailed within 5-10 business days
*You are required to take a refresher course every three (3) years. Contact your facility for more information.
To obtain a duplicate copy of your Medication Aide Certificate you must mail to VCU Department of Gerontology the following:
- A complete Medication Aide Certificate Request Form
- Check or Money Order for $7.00 made out to the Department of Gerontology. (Do Not Send Cash)
- Certificate will be mailed within 5-10 business days
